About The Program

The Diabetes Prevention Program focuses on lifestyle modifications. Individuals will learn throughout the program how to make and sustain lifestyle changes to help prevent or delay diabetes with the assistance of a trained lifestyle coach who will provide participants the help and support. The Diabetes Prevention Program is a yearlong program that meet weekly for one-hour session for 6 months. During the second 6 months, you will meet up to twice a month for one-hour session. Participants will learn how to adapt to a heathier lifestyle. Topic include: eating healthier, self monitoring of weight, adding physical activity to your life, coping with stress, managing life challenges, and tracking your progress. Research has found that after 10 years, people who have completed a diabetes prevention lifestyle progra, were 33 % less likely to develop diabetes.

Participant will learn to:

  • Eat healthy
  • Manage Stress
  • Add physical activity to your life
  • Understand and respond to your food cues
  • Stay motivated
  • Solve problems that can get in the way of healthy changes


The Diabetes Prevention Program is open to individuals who meet the following requirements:

  •  At least 18 years old and not pregnant at the time of enrollment
  •  Overweight (body mass index equal to or greater than 25;equal to or greater than 23 if Asian)
  •  No previous diagnosis of Type 1 diabetes

Meet one of the following:

  • Have a blood test result in the prediabetes range within the past year
  • Hemoglobin A1C: 5.7%-6.4 % or
  • Fasting plasma glucose: 100-125
  • Two-hour plasma glucose (after 75 gm glucose load): 140-199 mg/dL
  • Be previously diagnosed with gestational diabetes
  • Screen positive for prediabetes based on the CDC Prediabetes Screening Test

About Us

Tonya Health and Wellness is a for profit organization, and is in partnership with Southeast Michigan Diabetes Prevention Network organization working together to build the infrastructure for nationwide delivery of an evidence-based program for adults with prediabetes to prevent or delay onset of type 2 diabetes.

Tonya Health and Wellness is a Nurse Practitioner own business that offers a CDC approved National Diabetes Prevention Program. A key part of the National DPP is a lifestyle change program that provides a trained lifestyle coach, a CDC -approved curriculum, and group support over the course of a year. Individual can take the course in person or online.

Our mission is to educate and support individuals that want to live a healthier lifestyle.


To qualify for this program, individuals must be 18 or older, have a BMI of 25 or greater, be at a high risk for developing type 2 diabetes, or have been diagnosed with prediabetes by a physician. You can also take the Diabetes Risk Test to determine your personal risk. For more information on classes or to see if you qualify, email

This program is a prevention program for those who have been diagnose with prediabetes or may be at risk of developing type 2 diabetes. If you are currently living with diabetes, the Diabetes Prevention Center can connect you to othe resources to help you manage your diabetes, loke Diabetes Self-Management Education (DSME) or the National Kidney Foundation of Michigan's Diabetes-PATH program.

The Diabetes Prevention Program is a year-long program that consists of 16 weekly sessions, follow by bimonthly sessions for added support to help maintain progress. Each session meeting is an hour long.
